What Happened
Case management visit for unusual incident. Child left alone in bathroom for 3-5 minutes after staff member returned to classroom without realizing child was still in restroom.
Observations
- Child (C1) left unattended in hallway bathroom for approximately 3-5 minutes
- Staff member (S1) took children to bathroom and returned to classroom without accounting for all children
- Another staff member (S2) discovered child alone in restroom
- Violation of Title 22 Regulation Section 101229 (a)(1) regarding care and supervision
