What the inspector wrote

On October 29, 2024 at 9AM, Licensing Program Analyst (LPA) William Chancellor arrived unannounced to the facility to conduct a case management regarding an unusual incident report received on 10/8/24. The incident involved a child (C1) being left in a classroom for approximately two minutes during a transition outside. LPA met with Director (DIR) Desiree Buchanan and conducted a tour of the facility. Appropriate ratios and supervision were observed. During the visit, LPA conducted an interview with DIR, made observations of the classroom and collected documents regarding the incident. Interview and records reviewed, revealed that staff were in ratio at the time of the incident and C1 stayed behind in class while class transitioned outside. Records reviewed, confirmed that C1 remained inside while children were counted a second time outside, two minutes after exiting the classroom. DIR corroborated that C1 was found in a nook of the classroom by DIR who immediately escorted C1 outside. LPA has determined that the facility staff took the necessary steps to immediately return C1 to the playground with their classroom. Additionally, facility notified the parents and provided training to all staff, to improve the efficiency of name to face policies and ensuring a sweep of the classroom occurs, prior to exiting. Based on the information gathered, there appears to be no violations of Title 22 Regulations found at this time, and therefore, there were no deficiencies cited during this inspection. An exit interview was conducted, a copy of this report, LIC 811 (Confidential Names List) and appeal rights were reviewed with and provided to Director Desiree Buchanan. A notice of site visit was also provided and must remain posted for 30 days.
