Licensing Program Analyst (LPA) Otsanya Cameron arrived at the facility on a case management visit to follow-up on an unusual incident report submitted by the Dept on 8/10/21. LPA toured the facility, took census, and met with Director Shoneil Wilson to discuss the reported incident. During the visit, LPA reviewed training agenda for staff training held on 8/13/21 No violations of any regulation have been identified pertaining to the incident, however technical assistance regarding reporting requirements was provided during this visit. An exit interview was conducted and appeal rights were discussed with Director Shoniel Wilson and Notice of Site visit given. LPA observed posting.
