Licensing Program Analyst (LPA) Keely Messerschmidt arrived at the facility on a case management inspection to follow-up on an Unusual Incident Report (UIR) which occurred on 4/17/25 per Director. LPA met with Director Shoneil Wilson, and provided purpose of inspection. At the time of inspection, LPA toured the facility, took census, and reviewed documents previously submitted to the department with Director. Based on interviews conducted, it was disclosed that Child #1 (C1) had ran out of the preschool classroom and out of the infant classroom into the parking lot 2 times within a few minute time frame. It was also stated that someone outside grabbed C1 both times bringing C1 to Staff #1 (S1) at the doorway of infant classroom. LPA observed video footage that C1 ran out of preschool classroom 3 times and S1 brought C1 back to Staff #2 (S2) each time. LPA also observed S2 to leave preschool classroom 2 times looking for C1, leaving 3 other children alone without supervision. Lastly, LPA observed in the video footage S2 leaving a child in the bathroom alone unsupervised. Director stated that S2 no longer works at facility. Based on interviews conducted and video evidence the preponderance of evidence standard has been met. Therefore, a Type A citation will be issued for CCC Regulation 101229(a)(1) Responsibility for Providing Care and Supervision. See LIC809D page. Upon receipt, licensee shall post and provide copies of this licensing report to parents/guardians of children in care at the facility and to parents/guardians of children newly enrolled at the facility for the next 12 months. An exit interview was conducted and a copy of this report was provided. Appeal Rights were discussed and provided. The Notice of Site Visit and Type A Deficiencies from today’s visit must be posted for 30 days. Failure to keep these posted for the entire 30 days will result in an immediate $100 civil penalty for each.
