What the inspector wrote

Licensing Program Analysts (LPA), Kelli Waters and Sumayya Habeebulla conducted a Case Management visit on 05/15/24 to follow up on an Unusual Incident Report (UIR) that was submitted to Licensing by the facility on 05/02/24. LPAs met with Eunice Herrera, Site Director, and Cynthia De Dios, Program Director, to discuss incident. The Director reported the following; on May 1 st , 2024, C1 was left unattended outside on playground for approximately 4 minutes. During interviews with staff and video footage of the incident, Classroom B6 returned to the classroom with six children and three staff, not realizing that the seventh child was still on the playground. Four minutes later, parent of C1 noticed C1 in the playground area unsupervised. Parent did not observe any staff present outside. Parent then walked C1 to classroom B6. Staff of B6 was unaware that C1 was missing. See LIC 809-D for deficiency cited. An exit interview was conducted, and a copy of this report was provided. A copy of this report must be made available to the public, at the facility site, for 3 years.
