Census continued from page 1: Building #2: Toddler A had 9 toddlers with staff members Valerie Isham and Chalotte Cunningham Toddler B (outside on playground) had 7 toddlers with staff members Sarah DeLeon and Amanda Imhoff Toddler C had 11 toddlers with staff members Claudia Nelson, Carissa Velasquez and Jensen Purser During this visit, LPAs observed morning playtime, interviewed several preschool children and interviewed Director. It was alleged that the facility did not provide accident reports to the parents, the facility was out of ratio and staff did not ensure a safe environment for the children. During the course of the investigation, LPAs conducted interviews with the reporting party, staff members, parents of enrolled children and children at the facility. Parents interviewed stated that the facility was consistent about providing accident reports. LPA Lane reviewed sign in/out documents and staff timecards during the month of the initial complaint and was not able to substantiate the facility was out of ratio according to the documents as it appeared there was sufficient staff for the number of children enrolled. LPAs did not observe any ratio issues while at the facility today. Staff interviewed stated consistent incident report procedures. LPAs did not observe any hazards or issues that would make the environment unsafe for the children today. Although the allegations may have happened or are valid, there is not a preponderance of evidence to prove the alleged violations occurred, therefore the above allegations have been found to be UNSUBSTANTIATED. LPAs did become aware of an issue with a couple of children playing games involving pretending to hurt other children or teachers on the playground. LPAs spoke to Director regarding the issue and she stated parent conferences have been held and staff are continuing to work on the issue proactively with the children. LPAs provided technical assistance for additional resources regarding challenging behaviors and advised staff to continue to notify parents involved, document the incidents and closely monitor, shadow and supervise children regarding these issues. No deficiencies were cited at today's inspection. Exit interview conducted and report was reviewed with facility representative Director Sara Lee. A notice of site visit was provided and must remain posted for 30 days.
