What the inspector wrote

Based on the information obtained during interviews and documentation reviewed it is determined that the allegation is valid because the preponderance of the evidence has been met, therefore, the allegation is found to be SUBSTANTIATED. See LIC9099D for Type deficiency cited. Exit interview conducted and report was reviewed with the assistant center director, Dana Davis-Thaggard. A Notice of Site Visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. Appeal rights were provided.
