Inspector’s narrative

What the inspector wrote

On 02/10/2026 at approximately 09:30 AM, Licensing Program Analyst (LPA) Julieta Abrego made an unannounced case management visit to the facility to follow up on a self-reported inc ident that occurred on 02/03/2026, wherein a staff member was allegedly observed aggressively handling a child in care. It was reported that the staff member pulled a child from a chair, dragged the child by the wrist, and threw the child onto the floor inside the Honeycomb classroom. LPA met with the director to discuss the incident and obtain additional information. There were 13 children present with four (4) staff present during the inspection. There are currently 16 children enrolled in the daycare. LPA interviewed the directo r and attempted to interview two children; however, the children did not want to speak to the LPA . LPA obtained video footage of the incidents that occurred on 01/28/2026 and 02/03/2026. LPA also obtained a copy of the children\342\200\231s roster and reviewed staff and children files. It was reported that on 01/21/2026 the staff member grabbed the child by the wrist and dragged the child toward the playground, with the child\342\200\231s feet reportedly dangling and dragging on the flo or. The director and a parent observed the playground incident during a parent visit; however, there is no video footage of the outdoor incident because the cameras were not charged on that day. In another incident on 01/28/2026, the staff member was observed placing her hand on a child\342\200\231s head and neck, possibly forcing the child to nap . The staff member is no longer working at the facility as of 02/06/2026. Additionally, video footage from the staff member\342\200\231s last day of employment was provided to the LPA and will be reviewed at a later time. Further investigation is needed regarding the incident that occurred on 02/03/2026 and the additional incidents observed on video. An exit interview was conducted, and the report was reviewed with the director Mary Canales. A Notice of Site Visit was provided and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. No deficiencies were cited during today\342\200\231s visit.
