What the inspector wrote

On 1/6/26 at 1:22 p.m, Licensing Program Analyst (LPA), Renita Rodriguez conducted an unannounced case management site inspection. Upon arrival, LPA met with Acting Director Danielle Taglienti and Director Marla Martin. The purpose of this visit is to follow up on a self reported incident. Ratios observed: 46 children and 8 staff. An Incident occurred on 12/18/25 between 9:45 a.m-9:50 am. Child C1 used a wooden spool to step onto a plastic shed. Staff S1 and S2 observed child standing on shed and alerted staff. S1 started to walk over to child and child jumped off the shed onto ground containing wood chips. S1 states child was crying. Child had scrapes on side of the face from hitting the wood chips. Parent was notified and decision for child to stay to complete the day was made by parent. Child was taken to emergency room 12/19/25 due to child vomiting. The child was kept home and returned to center 1/5/26. The ratios on 12/18/25 at the time of the incident was 7 staff and 42 children. The center was closed from 12/24/25 thru 1/4/26. The child returned to the center on 1/5/26 with no medical directives. No deficiencies cited at this time. Exit interview and report reviewed with Director Marla Martin. Notice of Site Visit was provided and informed Director it shall remain posted for 30 days.
