What the inspector wrote

On 7/7/2025, at 2:01pm., Licensing Program Analysts (LPAs) Vicky Williamson and Oscar Picazo conducted an unannounced case management inspection during another inspection at the facility. LPAs met with Facility Representative, Cynthia Quintana, Assistant Director, Verenicia Chavez and Administrator, Celynn Preston. LPAs discussed the purpose of the inspection. There were 28 children present with seven (7) staff members. During the inspection, LPAs observed Staff 1 (S1) in classroom 3A supervising six children. Per Facility Representative, S1 has been employed at the facility since 11/15/2024 and does not have a criminal record clearance. S1 was removed from the classroom and left the premises during the time of inspection. Based on interviews and record review, one (1) Type A deficiency of California Code of Regulations, Title 22, Division 12, Chapter 1, is being cited on the attached LIC 809D. A $500 Civil Penalty was issued. LPA s informed Facility Representative, Cynthia Quintana that this report dated 7/7/2025 documents one (1) Type A citation, which shall be posted for 30 consecutive days as there is an immediate risk to the health, safety, or personal rights of children in care. Also, LPAs informed Facility Representative, Cynthia Quintana to provide a copy of this licensing report dated 7/7/2025 that documents a Type A citation to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification. LPAs provided Facility Representative, Cynthia Quintana with form LIC 9224. A Notice of Site Visit (LIC 9213) was given to Facility Representative, Cynthia Quintana and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days. Appeal Rights (LIC 9058) were provided. An exit interview conducted, and report was reviewed with Facility Representative, Cynthia Quintana
