What the inspector wrote

On 9/30/2025, at 9:00am, Licensing Program Analysts (LPAs) Vicky Williamson and Jacqueline Macias conducted an unannounced Case Management inspection regarding a self reported incident. LPA met with Facility Representative, Cynthia Quintana and disclosed the purpose of the inspection. LPAs accompanied by Facility Representative toured the indoor and outdoor of the facility. Days and hours of operation are Monday \342\200\223 Friday, 6:00am \342\200\223 6:00pm. There were 30 children present with four (4) staff members. Facility Representative reported that on 9/25/2025 at approximately 5:00pm, there were two teachers supervising 12 children on the playground. Staff #1(S1) and Staff #2 (S2) transitioned into the inside of the facility without conducting a name to face count of the children resulting in Child #1 (C1) being left alone on the playground without supervision. Staff #3 (S3) stated that she entered the Pre-K classroom to return cleaning supplies, when she heard a knock on the door leading to the playground. S3 stated that she opened the door and observed C1 outside alone on the playground. S3 stated that she observed C1 to be safe and unharmed. S3 stated that C1 was immediately taken back to the classroom and the incident was reported to Facility Representative via text message. It was determined that S1 and S2 were not aware that C1 was not inside of the classroom with the group of children that transitioned from the playground to the classroom. S1 and S2 acknowledged that C1 was left unsupervised on the playground for approximately 1- minutes. Facility Representative stated that C1\342\200\231s authorized representative arrived to the facility and was notified of the incident at that time. Interviews were conducted with the facility representative, staff and C1. C1\342\200\231s authorized representative was unavailable for interview. Video footage of the playground area was not captured on the day in question. See LIC 809C Continuation... LPA reviewed staff and children files, obtained copies of facility roster, sign in/sign out sheet, and related documentation. Based on interview statements, it was determined that C1 was left alone on the playground without supervision for 1-2 minutes. Facility Representative stated that supervision procedures were discussed and reviewed with S1 and S2; however, there will be a staff meeting scheduled with all staff in regards to supervision, name-to-face procedures and transitioning procedures. Per California Code of Regulations, Title 22, Division 12, Chapter 1, a deficiency is being cited on the attached LIC 809D. An immediate civil penalty of $500 was assessed for absence of supervision. LPA informed Facility Representative, Cynthia Quintana that this report dated September 30, 2025, documents one (1) Type A citation, which shall be posted for 30 consecutive days as there is an immediate risk to the health, safety, or personal rights of children in care. Also, LPA informed Facility Representative, Cynthia Quintana to provide a copy of this licensing report dated September 29, 2025, that documents a Type A citation to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification. LPA provided Facility Representative, Cynthia Quintana with form LIC 9224. A Notice of Site Visit (LIC 9213) was given to Facility Representative, Cynthia Quintana and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days. LPAs observed LIC 9213 was posted. Appeal Rights (LIC 9058) was provided. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. An exit interview was conducted, and the report was reviewed with the Facility Representative, Cynthia Quintana.
