What the inspector wrote

On December 16, 2025, at 9:30AM, Licensing Program Analyst (LPA) Vicky Williamson conducted an unannounced Case Management inspection to follow up on a self - reported incident. LPA met with Facility Representative, Cynthia Quintana and disclosed the purpose of the inspection. LPA accompanied by Facility Representative toured the indoor and outdoor of the facility. There were 26 children present, with four (4) staff members. Facility Representative Celynn Preston reported that on December 2, 2025, between approximately 3:28PM and 4:00PM, there was an alleged absence of supervision involving Child #1 (C1). Interviews were conducted with the facility representative, staff and daycare child in question. LPA reviewed staff files, and obtained copies of facility roster, sign in/sign sheet and related documentation. A notice of site visit (LIC 9213) was given to Facility Representative, Cynthia Quintana and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days. LPA observed LIC 9213 was posted. Appeal Rights (LIC 9058) were provided. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. Exit interview was conducted, and the report was reviewed with the Facility Representative, Cynthia Quintana.
