What the inspector wrote

On May 7, 2026, at 10:40 AM, an office meeting was held at the San Diego Child Care Regional Office to discuss the lack of supervision and criminal record clearance deficiencies. In attendance were Licensee Representative Edgard Manimtim, Office Administrators Celynn and Curtis Preston, Center Director Verenicia Chavez, Center Director Jessica Cabalos, Licensing Program Managers (LPMs) Tulam Vu, Selina Siao and Licensing Program Analysts (LPA) Vicky Williamson and Saray Siqueiros-Frayre. On September 30, 2025, the facility received a Type A deficiency under Section 101229(a)(1), Responsibility for Providing Care and Supervision. The citation stemmed from an incident on September 25, 2025, in which a four-year-old child in care was left unsupervised on the playground for approximately one minute. On February 17, 2026, the facility received another Type A deficiency under Section 101229(a)(1), stemming from an incident on December 2, 2025. In this incident, a four-year-old child in care was left alone in a bathroom inside the facility while the rest of the classroom was outside on the playground. The child was unsupervised for approximately one minute. On July 7, 2025, the facility received a Type A deficiency under Section 101170(e)(1), where a staff member did not have a criminal record clearance prior to employment. The facility promptly corrected all deficiencies. The facility submitted plans of correction to the Department, which included a summary of relevant licensing training videos. The plans indicated that staff were retrained on transition procedures, name-to-face protocols, staff positioning, the use of additional support staff, and bathroom and playground supervision. A staff meeting was held to reinforce supervision requirements, and a training verification form signed by participating staff was provided. The facility has signed up and implemented the Guardian System to verify that employees have completed the criminal record clearance prior to employment. During the office meeting, the Licensing Program Manager (LPM) and Licensing Program Analyst (LPA) reviewed the serious deficiency with the Licensee Representatives. The discussion emphasized the importance of proper transition procedures and compliance with Section 101229, Responsibility for Providing Care and Supervision and 101170 Criminal Record Clearance, to prevent future violations that could result in civil penalties. Technical Support Program (TSP) services and training opportunities were reviewed with the Licensing Representatives, and they were advised to register for Provider Information Notifications (PINs). Licensee Representatives were advised that repeated violations or failure to comply with licensing laws and regulations may result in administrative action taken by the Department. The Licensee Representatives acknowledged the seriousness of the matter and affirmed their commitment to ensuring the facility remains in full compliance with all applicable laws and regulations.
