What the inspector wrote

On 01/14/2022 at 1:12 PM, Licensing Program Analyst (LPA) Dana Stevens,made an unannounced complaint inspection to deliver findings on the above allegation. LPA met with Licensee, Tim Captain.Current census is 25. This agency has investigated the complaint alleging Child received a red mark on face while in care. During the investigation, the facility was inspected, interviews were conducted with facility staff, parents and records were reviewed. Department received informationt that on 11/15/2021, parent of Child 1 (C1) noticed a red mark on C1's face. Parent did not ask staff about the mark at the time of the incident and did not take photos of the mark. During interviews, facility staff denied seeing mark on C1's face and denied knowledge of any falls or injuries. According to staff interviews and record review, appropriate supervision and ratios were maintained during the operating hours. Based on the information obtained during investigation there is not a preponderance of evidence to support the allegation Child received a red mark on face while in care, thus the allegation is deemed unsubstantiated. Exit interview was conducted and copy of this report was left with Director. Notice of Site VIsit must be posted for 30 days.
