What the inspector wrote

On March 3, 2026, 9:15 AM, Licensing Program Analyst (LPA) Vicky Williamson conducted an unannounced Case Management inspection while at the facility conducting another inspection. LPA met with Facility Representative Verenicia Chavez. There were 19 children present, nine of whom are 18 to 36 months, with six (6) staff members. LPA accompanied by Facility Representative toured the indoor and outdoor of the facility. At 9:30 AM while touring the outdoor playground, LPA observed nine (9) children ages 18 - 36 months playing on the preschool playground. The Department granted the facility a playground waiver on July 29, 2019 that permits the infant and toddler children to share the infant playground at different times and not the preschool playground. LPA observed the playground waiver posted at the facility. A review of children records verified that the children observed on the playground during the time of inspection are 2 years of age and older; however they are enrolled into the toddler program. LPA observed that the play structures on the preschool playground were observed to be age appropriate. Interviews were conducted with the Facility Representative and staff. California Code of Regulations, Title 22, Division 12, Chapter 1, deficiencies are being cited on the attached LIC 809D. A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with the Facility Representative Verenicia Chavez.
