What Happened
LPAs inspected a new outdoor activity space and found it large enough for the current capacity. During the same visit, they observed unsafe sleep items in cribs, including a pacifier with beads, a bottle, and a stuffed toy, which led to a Type A deficiency.
Observations
- New outdoor activity space measured 1,857.10 square feet and was sufficient for current capacity
- A sleeping child had a pacifier with a string of beads attached
