What the inspector wrote

This is an amended version of the original report created on 10/11/21. On 10/11/21 at 9:00 a.m., Licensing Program Analysts (LPAs), Rajani Goudreau and David Miller arrived at the facility to conduct a pre-licensing inspection. Upon arrival LPAs, met with the President, Jennifer House, and VP of Operations, Jennifer Grawvunder, and proceeded to tour the facility. The facility is requested a capacity of 150 children; serving ages 2 years old through Kindergarten; including 12 toddlers ages; 18 through 36 months. The program will operate in the following preschool classrooms:  #1, #2, #3, #4, #5, #6,#7, #8-Frogs, #9, #10, Owls #11, #12, #13, #15, Honey Bee's #16 . Facility plans to operate Monday through Friday from 6:00 a.m. to 6:00 p.m. The following classrooms were inspected and measured during this inspection and measured the following: Room #1: measured a total of  476.81 square feet Room #2: measured a total of 308.82 square feet Room #3: measured a total of 131.15 square feet Room #4: measured a total of 318.47 square feet Room #5: measured a total of  373.87 square feet Room #6: measured a total of  139.89 square feet Room #7: measured a total of 492.54 square feet Frogs room #8 room: measured a total of 704.56 square feet Room #9: measured a total of 367.13 square feet Room #10: measured a total of 139.2 square feet Owls room #11 room: measured a total of 456.32 square feet Room #12: measured a total of 131.15 square feet Room #13: measured a total of 140.7 square feet Room #15: measured a total of  853.07 square feet Honey Bee's Room #16 (18 months through 36 months): measured a total of 563.75 square feet. Hallway #1 (Preschool area): measured a total of 398 square feet. Hallway #2 (Preschool area): measured a total of 491.04 square feet. See LIC809-C continuation page... This is an amended version of the original report created on 10/11/21. The total indoor space measures 6,485.47 square feet which is sufficient to accommodate 229 children at one time. However, due to insufficient toilets, the applicant is requesting a capacity of 150 children (138 preschool and 12 toddler children), updated LIC200A - Application received during the visit. The classrooms are equipped with appropriate furniture and napping equipment (cots), including tables, chairs, cubbies, bookshelves, and other materials for the children. There is a total of 10 restrooms with 10 toilets and 10 sinks. The toddler restroom is located inside the Honey Bee's #16 room (Toddler Option).   In addition, there is one sink in room #1, one sink in room #7, and one sink in the Honey Bee's room #16. The number of toilets and sinks is sufficient to accommodate 150 children. Children are to be escorted and supervised by staff to and from the restrooms and the playground areas and direct visual observation is required. Drinking water is available in the classrooms via refillable water jugs and cups provided by the facility. There is one staff restroom equipped with one sink and one toilet and is designated for ill children, if needed. The directors office will be used as an isolation area for ill children. There is a functioning carbon monoxide detector in the facility and a first aid kit with required supplies. Disinfectants, cleaning solutions, and other hazardous substances shall be kept inaccessible. There is a kitchen area that will be used as a food preparation area and includes three sinks, one refrigerator, one freezer, a stove, food supply, and storage units. The menus are posted in the classrooms. The center will be utilizing digital sign in and sign out. Required documents are posted in the lobby of the facility. The Preschool play yard area was inspected and measured a total of 16,071.47 square feet which is sufficient for 214 children. In addition, the toddler play area was inspected and measures a total of 1,130.8 square feet, which is sufficient for 15 toddlers. The pre-school play yard is fully fenced and includes outdoor play equipment; including a play structure and swings suitable for age groups 2 through 12 years old, and sand for cushioning, a shaded area including a shade kite. The toddler play yard is fully fenced with age appropriate play materials and with a building owning/kite for shade. Drinking water is readily available during outdoor activities via refillable water pitchers and cups provided by the facility. Proper supervision and placement of staff on the playground was advised to ensure the supervision and the safety of children. Incidental Medical Services (IMS) policy was discussed. For IMS information see Evaluator Manual - Regulation Interpretations and Procedures for Child Care Centers Section 101173 and 101226. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. The following information regarding ADA was provided:  US Department of Justice (USDOJ) toll-free ADA Information Line at (800) 514-0301 (voice/ (800) 514-0383 (TTY) and link to publication: Commonly Asked Questions about Child Care Centers and the ADA, available at: http://www.ada.gov/childqanda.htm . Services are in place. A fire clearance is pending from the fire Marshal. Fire inspection was completed on October 05, 2021 with no corrections. An exit interview was conducted with applicant representatives. The following reports were discussed and provided: LIC809, LIC809-C and Appeal Rights (LIC9058). A license will be issued for 150 children; serving ages 2 years old through Kindergarten; including 12 toddler age children; 18 through 36 months, after final file review. After final file review the license will be mailed to the facility and upon receipt shall be posed in a prominent place in the facility.
