On 05/17/22 at 10:00am, Licensing Program Analyst (LPA) Adrian Castellon conducted an unannounced inspection to deliver complaint findings for the above allegations: Facility is operating out of ratio, Day care child was left soiled for an extended period of time, Facility toilet does not accommodate day care children and Staff did not provide an incident report to authorized representative. LPA Castellon met with director Brenda Casillas and discussed the purpose of the inspection. During the course of the investigation, two unannounced inspections were conducted. Interviews were conducted with day-care parents, facility staff, and children in care. LPA Castellon obtained parent sign in and out sheets, class ratio sheets, and bathroom/diaper logs. LPA Castellon could not corroborate the allegations. Due to conflicting statements obtained during the course of the investigation, the above allegation is deemed to be UNSUBSTANTIATED meaning that although the allegation may have happened or is valid, there is not a preponderance of evidence to prove that the alleged allegation occurred. A copy of today's report, Notice of Site Visit and appeals rights given to the licensee. An exit interview was conducted with facility director and she stated that she understood. Notice of Site Visit should be posted for 30 days from today's date.
