On September 27, 2024, at 9:45am Licensing Program Analyst (LPA), Adrian Castellon, conducted an unannounced Case Management Inspection due to an incident that involved child #1. On 09/20/2024, the Department received the incident report and other documents from the facility regarding child #1 incident. The facility also telephoned the SDCCRO and LPA Castellon to report the incident on 9/20/24. During today's case management inspection, LPA met with director Brenda Casillas. Present during today's inspection were 100 children in care. The facility self reported on 9/20/2024 that C1's parent reported to Director that C1 advised parent that staff member hit her on the mouth while in care. On this date, LPA interviewed director and a co-staff member of staff member accused. LPA Castellon interviewed C1. LPA Castellon telephoned and interviewed C1's parent on this date. A classroom roster and C1's file was also obtained. Child has returned to facility. Staff member accused has been placed on leave. Further investigation is needed. No deficiencies issued during today's visit. LPA conducted an exit interview with the licensee.
