What the inspector wrote

Observation of the facility surveillance videos reveal a deceased rodent on a ceiling tile when the ceiling tile was removed by workmen. Based on LPAs\342\200\231 observations, conducted interviews and record reviews, the preponderance of evidence standard has been met, therefore the above allegation that facility staff did not ensure classroom was kept in a sanitary condition for children in care is found to be SUBSTANTIATED, California Code of Regulations, (Title 22, Division 12 Chapter 3) are being cited on the attached LIC 9099D.) LPA provided Director Prowant with a hard copy of CCR Title 22 Section 101238(a)(1) \342\200\234Buildings and Grounds\342\200\235. Director and LPA reviewed this regulation together. A copy of this report, notice of site visit (LIC 9213), and appeal rights (LIC 9058) was provided to Director Prowant. A notice of site visit was given and must remain posted for 30 days. LPA observed the notice posted. An exit interview was conducted, and report was reviewed with Director Tina Prowant. This report and a copy of their appeal rights (LIC 9058) was provided to Director Tina Prowant.
