What the inspector wrote

On 11/28/2023 at 02:56pm an office meeting was conducted with Licensee Representative, Michael Ong (present via Microsoft Teams) to discuss deficiencies cited on 09/29/2023, 09/06/2023, 01/4/2023 and 05/22/2022. Present at the meeting were Regional Director, Nichelle Gissler, Director, Krizia Beltran, Licensing Program Manager (LPM), Tulam Vu and Licensing Program Analyst (LPA), Vicky Williamson. On 09/29/2023, the facility was cited Type A and issued a civil penalty for a repeat violation, Section 101229(a)(1): A two year old daycare child was left unsupervised in the hallway of the facility for 1 to 2 minutes. On 09/06/2023, the facility was cited Type A, Section 101229(a)(1): A three year old daycare child was left on the playground unsupervised for 9 minutes. On 01/04/2023, the facility was cited Type B, Section 101238(a)(1). A rodent was found dead in the ceiling tiles inside of a classroom. On 05/10/2022, the facility was cited Type A, Section 101229(a)(1): A three year old daycare child was left on the playground unsupervised for an unknown length of time. The director corrected all deficiencies in a timely manner. Director stated that all staff members have received retraining and reviewed videos on Supervision on the CCL website. LPM discussed with Licensee Representative the above deficiencies. The following resources were provided: Technical Support Program (TSP) brochure, childcaretechnicalsupport@dss.ca.gov ., PIN 23-19-CCP, Advocate information childcareadvocatesprogram@dss.ca.gov., and the Duty Line 619-767-2248. Submit Unusual Incident Report by email to: SDIncidentReports@dss.ca.gov or by Fax at (619) 767-2203. Regulation Section 101229 Responsibility for Providing Care & Supervision and the Best Practices Lack of Supervision handout were provided. Licensee Representative was informed that repeated violations or failure to comply with licensing laws and regulations may result in administrative action taken by the Department. Licensee Representative stated they understood and will ensure the facility complies with all regulations and laws governing Child Care Centers.
