On 06/11/2025 Licensing Program Analyst (LPA) Dana Stevens conducted an unannounced case management inspection regarding a self-reported Unusual Incident. LPA met with Director, Krizia Beltran and discussed the purpose of the visit. Director accompanied LPA on a tour of the facility and preschool playground. The following ratios were observed: Pre-K 2 - 18 napping children (ages 3.5-5 years) with one teacher (one teacher on break) Pre-K 1 - 12 napping children (ages 3.5-6) with one teacher Preschool class - 14 napping children (2.5-3.5) with one teachers and one aide (one teacher on break) On 06/05/2025 Director submitted an Unusual Incident Report, reporting that information was received that inappropriate interactions occurred between two preschool children on or around 06/02/2025. Director stated that on 06/03/2025, Daycare Parent 1 (P1) reported to her that Child 1(C1) stated they witnessed inappropriate interactions between Child 2 (C2) and Child 3 (C3). On 06/05/2025, Director received additional information from Daycare Parent 2 (P2), that C2 also reported inappropriate interactions had occurred between C2 and C3 during recess on the playground. LPA interviewed Director, staff and daycare children and collected facility records. Director stated she will provide camera footage via email. This incident requires further review. Exit interview conducted and copy of this report and appeal rights were provided to Director, Krizia Beltran. Notice of Site Visit must be posted for thirty (30) days.
