What the inspector wrote

This is an amended version of the original report created on 12/31/21. On 12/31/21 at 8:00 a.m, Licensing Program Analyst (LPA), Rajani Goudreau arrived at the facility to conduct a pre-licensing (change of location) inspection. Upon arrival LPA, met with Licensee Representative, Letitia Arias and proceeded to tour the facility. The facility is requested a capacity of 20 preschool children; serving ages 2 through 6 years old. This preschool program will operate in the preschool classroom #1. Facility plans to operate Monday through Friday from 7:30 a.m. to 5:30 p.m. The following preschool indoor areas were inspected and measured during this inspection and measured the following: Preschool Classroom #1: (2 through 6 years old): measured a total of 616.56 square feet. Art/Sensory Area measured a total of 113.62 square feet. The total indoor space measures 730.18 square feet which is sufficient to accommodate 20 children at one time. Applicant agreed to lower the capacity to 20 children due to limited indoor space, updated LIC200A received at time of visit. The classroom & Art/Sensory is equipped with age appropriate furniture and play equipment; including napping equipment (mats), tables, chairs, cubbies, bookshelves, and other materials for the children use. The classroom and play area is orderly, free of hazards and has adequate heating, lighting and ventilation. The preschool program will utilize a restroom with exclusive use during facility operation hours located in the church area of the building, containing six sinks and eight toilets. The number of toilets and sinks is sufficient to accommodate the requested capacity of 20 children. Applicant will submit a plan of operation regarding the isolation stall in the children’s restroom and the transition of the children to and from the restroom. The restroom and hand washing area is in safe, sanitary and oper ating condition. Parents/Authorized Representatives will provide snack and meals for the children. Uncontaminated drinking water is readily available during indoor and outdoor activities through the use of refillable water containers and refillable water bottles brought from home. See LIC809-Continuation page.. There is a separate restroom for staff located in the church area of the building near the children’s restroom. The director’s office is designated for ill children, if needed. In addition, a stall in the children’s restroom will be designed for ill children. There is a functioning carbon monoxide detector. Disinfectants, cleaning solutions, and other hazardous substances shall be kept inaccessible. The center will be utilizing manual sign in and sign out. Required documents are posted in the front of the facility. The outdoor preschool play area was inspected and measures a total of 2,819.84 square feet which is sufficient for 37 children. The outdoor space is located in the far east side of the parking lot. Also, the children will have to go through the church area to access the outdoor space. Applicant will submit a plan of operation to ensure supervision is maintained during the transition of children to and from the outdoor activity space. The play area is fully fenced and includes outdoor play equipment, sufficient (wood chips) for cushioning around the slides and play structures and shade for the children by the use of shade kites. Partial of the plastic on the play structure slide is torn with sharp and loose edges. In addition, unable to determine if the large play structure is suitable for ages 2 through 6 years old. There is no evidence of rodent or insect activity. Proper supervision and placement of staff during outdoor activities was advised to ensure the supervision and the safety of children. This facility plans to provide Incidental Medical Services – IMS plan on file and approved. LPA and Applicant discussed California Megan's Law and LPA provided: www.meganslaw.ca.gov . A fire clearance for 21 Preschool children was granted on December 27, 2021. An updated fire clearance will be requested due to the decrease in the capacity request. See LIC809-C continuation page... The following is pending with a due date of January 03, 2022 : Plan of operation for the transition of children to and from the restroom and designated toilet stall for ill children Plan of operation for the transition of children to and from the outdoor activity space. Documentation of the play structure being suitable for ages 2 through 6 years old. Fix broken plastic on play structure slide. Provide first Aide kit with the required supplies An exit interview was conducted with Licensee Representative, Letitia Arias. The following reports were discussed and provided: LIC809 and Appeal Rights (LIC9058). Once pending documents are received and after final file review, a license will be issued for 20 Preschool children ages 2 through 6 in preschool classroom #1.
