What the inspector wrote

On July 16, 2025, at 8:25 a.m., Licensing Program Analyst (LPA), Angela Nguyen conducted an unannounced Plan of Correction (POC) inspection. The purpose of this inspection is to ensure that citations issued during the Required inspection on 02/24/2025 were corrected. LPA met with Facility Representative, Jacquelyn Aguigui- Farjardo and was led on a tour of the facility. There were two infants being supervised by 2 staff members. LPA verified that the following correction was completed: \342\200\242 Outdoor play area is free of hazards. \342\200\242 15-minute sleep logs documenting the time, observation and sleeping position. LPA inspected the outdoor play area and determined it to be free of hazards. LPA reviewed children sleep logs and determined it to be accurately documented within 15 minute intervals, with description of observation and sleeping position. A Notice of Site Visit (LIC 9213) was given to Facility Representative, Jacquelyn Aguigui- Fajardo and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days. LPA observed LIC 9213 was posted. Appeal Rights (LIC 9058) was provided. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. An exit interview was conducted and this report was reviewed with Facility Representative, Jacquelyn Aguigui- Fajardo.
