What the inspector wrote

On 11/04/22 at 9:00 a.m., Licensing Program Analyst (LPA), Rajani Goudreau arrived at the facility to conduct a pre-licensing inspection. Upon arrival LPA, met with applicant, Connie Serrano and proceeded to tour the facility. The facility is requested a capacity of 30 children; serving 18 preschool children; ages 2 through 5 years old in classroom #2; including 12 toddler children; ages 18 through 36 months in classroom #1.\302\240 Facility plans to operate Monday through Friday from 7:00 a.m. to 5:30 p.m. The following classrooms #1 and #2 were inspected and measured during this inspection and measured a total of 1,109.38 square feet, which is sufficient to accommodate 31 children at one time. However, facility is only requesting a capacity of 30 children. The classrooms are equipped with age appropriate furniture and play equipment; including napping equipment (cots and one crib for the toddler option), tables, chairs, cubbies, bookshelves, and other materials for the children. The classrooms are orderly, free of hazards and have adequate heating, lighting and ventilation. Classroom #1 and #2 each have a wall heater in the classrooms that will not be used and is made securely inaccessible. The preschool and toddler children will utilize one restroom outside near the corner of the west side building containing two sinks and two toilets for the children's use. Plan of operation shall be submitted for the shared usage of the restroom. There is one sink in classroom #1 and one sink in classroom #2 available for the children's use. Toddler classroom #1 has a changing table that meets regulations within arm's reach of the sink, per observation. There is a total of two toilets and four sinks available for children's use in the facility. The number of toilets and sinks is sufficient to accommodate the requested capacity of 30 children. The restroom and hand washing areas are in a safe, sanitary and operating condition, per observation. The facility will provide AM and PM snacks to the children. Breakfast and lunch will be brought from home. A kitchen will be utilized for snack preparation, containing one fridge, one sink and one stove. Uncontaminated drinking water is readily available during indoor and outdoor activities through the use of refillable water containers. The staff restroom located near the children's restroom will be designated for ill children. Each of the classrooms will have a designated isolation area, if a child becomes ill. There is a functioning carbon monoxide detector and a first aid kit with partial of the required supplies located in the classroom, per observation.\302\240 Disinfectants, cleaning solutions, and other hazardous substances shall be kept inaccessible to the children in care. The facility will be utilizing manual sign in and sign out. Required documents were observed to be posted. The outdoor play area was inspected and measured a total of 7,093.52 square feet which is sufficient for 94 children. The preschool and toddler children will share the outdoor space on a scheduled basis, per applicant. Playground waiver for shared outdoor space shall be requested from the applicant. The outdoor play is fully fenced and includes outdoor play materials that will be suitable for ages 18 months through 5 years old. There is no play structure in the outdoor play area. LPA observed a sand box available for the children's use. LPA informed applicant to ensure the sand box is inspected daily and free from hazardous foreign materials. LPA observed adequate shade in the outdoor play area by the use of building awnings. LPA observed the outdoor activity space surfaces to be free of hazards. There is no evidence of rodent or insect activity, per observation. Proper supervision and placement of staff during outdoor activities was advised to ensure the supervision and the safety of children during outdoor play. LPA reviewed with applicant the LIC 311A, Records To Be Maintained At The Facility, for child\342\200\231s records, personnel records, administrative records, and documents to be posted. Community Care Licensing Division (CCLD) regularly sends information to licensed facilities, providers, and stakeholders by way of Provider Information Notices (PIN), Program Quarterly Update Newsletters and other important information communication platform. Applicant was reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center.\302\240 A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated. This facility plans to provide Incidental Medical Services \342\200\223 IMS. IMS plan on file and approved. For IMS information, see PIN 22-02-CCP. A Plan of Operation that includes IMS must be submitted to the Department. LPA informed applicant initial water lead testing results shall be received and posted within 180 days of licensure. LPA provided and discussed the following with the applicant: PIN\342\200\231s 21-04-CCP, 22-06-CCP and 21-21-CCP, External Water Sampler Self-Certification Form (LIC 9275), Child Care Center Sampling Checklist Form (LIC 9276). A fire clearance for 30 children; including 18 preschool children; ages 2 through 5 years old in classroom #2, including 12 toddler children; ages 18 through 36 months children in classroom #1 was granted on October 13, 2022. The following is pending with a due date of November 07, 2022: Plan of operation for shared restroom usage Playground waiver request for shared outdoor space Picture of Tweezers and Thermometer for first aid kit An exit interview was conducted with applicant, Connie Serrano. The following reports were discussed and provided: LIC809 pages and Appeal Rights (LIC9058). After pending documents are received, review and approval of playground waiver and after final file review a preschool license will be issued for 30 children; including 18 preschool children; ages 2 through 5 years old in classroom #2, and 12 toddler; ages 18 through 36 months children in classroom #1. Once license is issued the license will be mailed to the applicant and once received shall be posted in the facility.
