Inspector’s narrative

What the inspector wrote

On September 24, 2025, at 9:30 a.m., Licensing Program Analyst (LPA), Angela Nguyen conducted an unannounced Required inspection. LPA met with Facility Representative, Erica Serrano and disclosed the purpose of inspection. LPA was led on a tour of the inside and outside the facility by Facility Representative. This is a full day program which operates year around schedule. Days and hours of operation are Monday - Friday from 7:00 a.m. to 5:30 p.m. The following ratios were observed: (7) children being supervised by (2) staff member in Classroom #1 and (6) children being supervised by (1) staff members in Classroom #2. Classroom #3 is currently not in use due to low enrollment. All toilets and hand washing facilities were observed to be in safe and sanitary operating condition. The floors in the facility were observed to clean and safe. Solid waste storage containers were observed to have tight-fitting covers and in good repair in all the classrooms. Drinking water is available both indoors and outdoors. The facility provides AM snack. Children bring their own water bottles and filter drinking water is available at the facility. Menus are posted weekly. LPA observed areas around high climbing equipment have artificial turf as cushioning in play areas to absorb falls. LPA observed the play equipment is age appropriate. Shade was observed in the outdoor activity space being provided by the shade kites. The facility has a functioning carbon monoxide detector and a first aid kit that meet statutory requirements. The last fire drill was conducted and documented on 8/4/2025. Facility Representative stated that there are no swimming pools or other bodies of water on the premises. Facility Representative stated there are no firearms or ammunition allowed or stored on the premises. Facility Representative was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated. During the inspection it was discovered that Connie Angelina Serrano had an active criminal record clearance however, was not associated to the facility. Capacity and limitations as specified on the license are being maintained. Children are under supervision, including visual supervision, of a teacher at all times. Facility maintains a ratio of one teacher supervising no more than twelve (12) children in care. At least one person trained in CPR and Pediatric First Aid is present when children are at the facility. The name of the facility representative and the fully qualified teacher designated to act in the facility representative\342\200\231s absence has been reported to the Department. LPA reviewed facility sign in /sign out sheets found to be complete. Facility utilizes an physical sign in/ out procedure. Facility representative was reminded that the person who signs the child in and out of the facility shall use their full legal signature and record the time of day. LPA reviewed a sample of children\342\200\231s files and observed files were complete with contact information for authorized representative and or relatives or others who can assume responsibility for the child and medical assessment. LPA reviewed a sample of staff files and observed files were complete with health screening, immunization records for influenza, pertussis and measles and current documentation of completed mandated reporter training however, three staff member did not have proof of education and one staff did not complete mandated reporter training certification. LPA verified that the lead testing was completed in accordance to the Written Directives outlined in PIN 21-21.1-CCP. LPA discussed the safe sleep regulations with Facility Representative and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed Facility Representative of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment. Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02-CCP. Facility Representative stated she will submit LIC 309, LIC 308, LIC 610, LIC 500 and LIC 9040 to the Department within 30 days. (2) Type B California Code of Regulations, (Title 22, Division 12 & Chapter 3), are being cited on the attached LIC 809-D. (2) Technical Violations were issued during this inspection. A Notice of Site Visit (LIC 9213) was given to Facility Representative, Erica Serrano and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days. LPA observed LIC 9213 was posted. Appeal Rights (LIC 9058) was provided. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. An exit interview was conducted and the report was reviewed with Facility Representative, Erica Serrano.
