Facility-requested visit·February 18, 2025

On 2/18/2025 at 10:46 am, Licensing Program Analyst (LPA), Selina Siao conducted an unannounced Case Management inspection for a decrease in capacity from 51 to 39 children. Upon arrival, LPA met with Director Virginian Andrade. Applicant is requesting to care for 39 preschool children ages 2 to 5 years old in classrooms #1, #2 and #3. Facility operates Monday to Friday from 6:30am to 5:30pm. Present during today’s inspection were 33 preschool children with 5 staff members in the three preschool classrooms. All staff members have the required background clearances and appropriate ratios, and supervision were observed. All indoor and outdoor activity space utilized for the children was inspected today. LPA informed Virginia Andrade that staff are required to maintain direct visual supervision of the children at all times during indoor and outdoor activities. LPA continued to tour the facility and remeasured all the indoor and outdoor activity space. Total indoor activity space measured to be 1345.25 square feet, which is sufficient to accommodate 38 children. LPA observed all indoor activity space to be complete with safe, age-appropriate furniture and equipment, including tables, chairs, cubbies, napping cots, bookshelves, and other activity supplies for the children. Drinking water is available in the classrooms via the children’s own water bottles and it gets refill by a water pitcher. Director stated that the water in the pitchers is from the kitchen sink faucet. LPA observed all hazardous items to be inaccessible to children. There are two fish tanks in the room with cover. There is no other bodies of water or weapons on the property. Fire clearance was granted on 1/22/2025. LPA observed a total of 4 sinks and 6 toilets available for the children’s use. These are sufficient to accommodate the allowable capacity of 38 children. The facility currently has a fully fenced playground area. The wooden fence and brick wall fencing is at least four feet high. The total square footage for all the outdoor activity space is 2873.47 square feet, which is sufficient to accommodate 38 children. Shade is provided via trees and sail shade. There are sufficient outdoor age-appropriate toys and play equipment available on the playground. There is a climbing structure on the playground that appears to be age appropriate for preschool children ages 2 to 5 years old. There is adequate cushioning in fall zones of climber provided by wood chips. Drinking water is available via the children’s water bottles and there is a small drinking fountain that is also available for children. LPA observed all hazardous items on the playground to be inaccessible to children. Director was reminded that any changes to the facility must be reported to and approved by Community Care Licensing. The following corrections are needed prior to the issuance of the license: Add room numbers for classrooms #1, #2 and #3. Licensee shall submit an amended application to request for 38 preschool children as the indoor activity space is insufficient for the requested capacity. Director understands that all proof of corrections must be provided to the Department within 30 days, or the application may be denied. A notice of site visit was given to Director Virginia Andrade and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days. Exit interview conducted and report was reviewed with the Director Virginia Andrade.
