What the inspector wrote

On 2/18/2025 at 9:35am, Licensing Program Analyst (LPA), Selina Siao conducted an unannounced Case Management inspection for an increase in capacity. Upon arrival, LPA met with Director Virginia Andrade. Applicant is requesting to care for 20 infants ages 6 weeks to 24 months in classrooms A and B (new room). Facility operates Monday to Friday from 6:30am to 5:30pm. Present during today’s inspection were 3 infants inside the classroom A supervised by Linda Skuce and 4 infants were outside the infant outdoor play area supervised by 1 staff. All staff members have the required background clearances and are associated to the facility. Appropriate ratios and supervision were observed. All indoor and outdoor activity space utilized for the children was inspected today. LPA informed Ms. Virginia that staff are required to maintain direct visual supervision of the children at all times during indoor and outdoor activities. LPA continued to tour the facility and remeasured all indoor and outdoor activity space. Total indoor activity space measured to be 638.67 square feet, which is sufficient to accommodate 18 infants and insufficient for the requested capacity. LPA observed all indoor activity space to be complete with safe, age-appropriate furniture and equipment, including tables, chairs, cubbies, napping cribs and cots and mats for infants who are able to climb out of the crib, bookshelves, and other activity supplies for the children. Drinking water is available in the classrooms via a water pitcher that gets refill from the kitchen sink faucet. LPA observed 5 cribs in the infant room A and 6 foldable cribs with wheels in Infant room B. LPA observed all hazardous items to be inaccessible to children. There are no bodies of water or weapons on the property. Fire clearance was granted on 1/22/2025. High chairs and feeding tables have broad-based legs, plastic seats are in good repair and trays lock onto chairs. High chairs are made of washable, moisture-resistant material. Changing tables have at least 1” padding covered with moisture-resistant, washable material. Sides of the changing table are at least 3” high and the changing table is within arm’s reach of a sink. Diapering sink is not used for meal preparation or dishwashing. Cribs meet regulatory requirements. Crib area is separated from activity space via a half wall. This barrier walls are 3 feet and 9 inches high for both rooms. Both barrier walls shall be extended to four feet high, and it is made of sound absorbing material and allows for supervision of napping children. Sleep logs are kept in the Brightwheel app. Needs and Services Plans and Sleep Plans are kept in a binder inside the individual classrooms. There shall be at least one potty chair for every five potty training infants. LPA observed a total of 2 sinks available for children’s use. These are sufficient to accommodate the requested capacity. There is a separate staff restroom equipped with a toilet and a sink. The isolation area for children who are ill will be the Director’s office and the isolation restroom is the staff’s restroom. The facility currently has a fully fenced playground area. The brick wall fencing is at least four feet high. The total square footage for all the outdoor activity space is 658.75 square feet for the two Infant yards combined, which is only sufficient to accommodate 8 infants at one time, insufficient for the number of children being requested. A playground waiver to share the playground at different times in the AM and PM due to limited square footage shall be submitted and approved prior to the capacity increase . Shade is provided via a tarp and patio cover and the building. There are sufficient outdoor age-appropriate toys and play equipment available on the playground. There is no climbing structure on the playground. There is turf at the infant yard area that is located right outside of the infant room A. Director stated that they are planning to add padding and turf at the infant yard #2. Drinking water is available via the children’s own zippy cups or water bottles when the children are outside. LPA observed all hazardous items on the playground to be inaccessible to children. Director Virginia Andrade was reminded that any changes to the facility must be reported to and approved by Community Care Licensing. The following corrections are needed prior to the issuance of the capacity increase: Add room numbers for each classrooms Infant A and Infant B. Add a signae to identify the Infant Sink located next to the new Infant B room. Extend the crib wall barrier to be at least 4 feet high. Licensee shall submit an amended application to request for 18 infants as the indoor activity space is not sufficient for the requested capacity. Director Virginia Andrade understands that all proof of corrections must be provided to the Department within 30 days, or the application may be denied. Exit interview conducted and report was reviewed with the Director Ms. Virginia Andrade.
