What the inspector wrote

On 11/08/22 at 9:00 a.m., Licensing Program Analyst (LPA), Rajani Goudreau arrived at the facility to conduct a pre-licensing (change of ownership) inspection. Upon arrival LPA, met with applicant, Nicole Hall and proceeded to tour the facility. The facility is requested a capacity of 59 preschool children; serving ages 2 years old through kindergarten in the following classrooms: Palm, Pine and the Office/Kindergarten room. Facility plans to operate Monday through Friday from 7:00 a.m. to 6:00 p.m. The classrooms were inspected and measured during this inspection and measured a total of 2,089.45 square feet, which is sufficient to accommodate 59 children at one time. Based on indoor space facility is requesting to change requested capacity from 50 to 59 children. LPA received updated LIC200A-Application at time of inspection. The classrooms were observed to be equipped with age appropriate furniture and play equipment; including napping equipment (mats), tables, chairs, cubbies, bookshelves, and other materials for the children use. Regarding the office/kindergarten classroom, only the kindergarten classroom area is used for the children with the office separated by a room divider. The classrooms are orderly, free of hazards and has adequate heating, lighting and ventilation. There are three sinks and a restroom area containing two toilets in the Pine classroom available for the children\342\200\231s use. In addition, there are two sinks and a restroom area in the palm classroom including two toilets available for the children\342\200\231s use. Restrooms located in the classrooms were not calculated in the square footage of the indoor space. The number of toilets and sinks is sufficient to accommodate the requested capacity of 59 children. The restrooms and hand washing areas are in a safe, sanitary and operating condition, per observation. The facility will provide AM and PM snacks to the children. Breakfast and lunch will be brought from home. Uncontaminated drinking water is readily available during indoor and outdoor activities through the use of refillable water pitchers and disposable cups provided by the facility. The staff restroom located in the Palm classroom will be designated for ill children. The director\342\200\231s office will be designated as an isolation area for ill children. There is a functioning carbon monoxide detector and a first aid kit with required supplies located in the facility, per observation.\302\240 Disinfectants, cleaning solutions, and other hazardous substances shall be kept inaccessible to the children in care. The facility will be utilizing manual sign in and sign out. Required documents were observed to be posted near the front entrance of the facility. The outdoor preschool play area was inspected and measured a total of 5,643.79 square feet which is sufficient to accommodate 75 children at one time. The outdoor play area is fully fenced and includes outdoor play equipment that is age appropriate. There is sufficient rubber mulch material for cushioning under and around the play structure that absorbs falls; including swings and a slide. LPA observed adequate shade in the outdoor play area by the use of canopy\342\200\231s. LPA observed the outdoor activity space surfaces to be free of hazards. There is no evidence of rodent or insect activity, per observation. Proper supervision and placement of staff during outdoor activities was advised to ensure the supervision and the safety of children during outdoor play. LPA reviewed with applicant the LIC 311A, Records To Be Maintained At The Facility, for child\342\200\231s records, personnel records, administrative records, and documents to be posted. Community Care Licensing Division (CCLD) regularly sends information to licensed facilities, providers, and stakeholders by way of Provider Information Notices (PIN), Program Quarterly Update Newsletters and other important information communication platform. Applicant was reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center.\302\240 A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated. This facility plans to provide Incidental Medical Services \342\200\223 IMS. IMS plan on file and approved. For IMS information, see PIN 22-02-CCP. A Plan of Operation that includes IMS must be submitted to the Department. LPA informed applicant initial water lead testing results shall be received and posted within 180 days of licensure. LPA provided and discussed the following with the applicant: PIN\342\200\231s 21-04-CCP, 22-06-CCP and 21-21-CCP, External Water Sampler Self-Certification Form (LIC 9275), Child Care Center Sampling Checklist Form (LIC 9276). The following is pending, prior to the licensure of the facility: Classroom identification. A fire clearance for 50 Preschool children; ages 2 years old through kindergarten in the Palm, Pine and Office/Kindergarten classrooms was granted on October 27, 2022. An updated fire clearance will be requested due to change in capacity from 50 to 59 children. An exit interview was conducted with applicant, Nicole Hall. The following reports were discussed and provided: LIC809 pages and Appeal Rights (LIC9058). Once pending document and updated fire clearance is received and after final file review a preschool license will be issued for 59 children; ages 2 through kindergarten in the Palm, Pine and Office/Kindergarten classrooms. Once license is issued the license will be mailed to the applicant and once received the license shall be posted in the facility.
