What the inspector wrote

On 08/16/2022 at 9:28 a.m., Licensing Program Analyst (LPA), Rajani Goudreau arrived at the facility to conduct a pre-licensing, change of location inspection. Upon arrival LPA, met with the Director of Special Programs, Shane Schmeichel and proceeded to tour the facility. The facility is requested a capacity of 72 children; serving ages 3 through 5 years old in classrooms #701, #709 and #710 at Village Elementary. Facility plans to operate two sessions following the school districts calendar: Monday through Friday from 8:15 a.m. to 11:15 a.m. and 12:15 p.m. to 3:15 p.m. This facility is a Title-5 program. The classrooms are located in building 700. The classrooms were inspected and measured during this inspection and measured a total of 2,830.79 square feet. Total indoor space is sufficient to accommodate 80 children at one time. The classrooms are equipped with age appropriate furniture, including tables, chairs, cubbies, bookshelves, and other materials for the children. Classroom #701 has two sinks and two restrooms in the classroom, each containing one toilet. In classroom #709 there is two sinks and two restrooms in the classroom, each containing one toilet. Also, in classroom #710 there is one sink in the classroom. Two restrooms will be available for the children's use located in building 700; one restroom containing three sinks and three toilets and the other restroom containing three sinks, one toilet and two urinals.The two restrooms located in building 700 will be shared with the school age children. There is sufficient toilets and sinks based on the requested capacity. Applicant understands a waiver will be needed due to the shared restroom usage. Classroom #709 will be shared with the transitional kindergarten (TK) program. Classroom #710 will be shared by the TK and kindergarten school programs. The preschool program will utilize the classrooms during a time the school programs are not in session. Plan of operation shall be submitted for the shared classroom usage. See LIC809-Continuation page. The nurse\342\200\231s office and nurse\342\200\231s restroom will be used as an isolation area for ill children. There is a functioning carbon monoxide detector and a first aid kit with required supplies. Disinfectants, cleaning solutions and other items that could pose a danger shall be stored where they are inaccessible to children. Breakfast, Lunch and Snack will be provided by the school. The center will be utilizing Digital sign in and sign out. Required documents are posted. The outdoor play area is located adjacent to the classrooms. The outdoor play area was inspected and measures a total of 3,115.58 square feet which is sufficient for 41 children. The surface of the outdoor activity space is maintained in a safe condition and free of hazards. Play structure has cushioning material to absorb falls by the use of sufficient rubber matting. There is shade in the outdoor playground by the use of a movable canopy and shade kites over the play structure. The play equipment is suitable for ages 2 through 5 years old. Proper supervision and placement of staff on the playground was advised to ensure the supervision and the safety of children. Drinking water is available both indoors and outdoors by the use of water fountains. The outdoor play area is fenced, however, there is no barrier between the preschool and schools outdoor play area. In addition, outdoor play area will be shared by the TK and Kindergarten programs on a scheduled basis. Applicant understands a waiver will be needed due to the limited outdoor space to accommodate the requested capacity and the shared outdoor space. Also, a plan of operation included in the waiver request reflecting the plan to ensure there is no co-mingling of the preschool and school aged children and supervision in place during outdoor activities. This facility provides Incidental Medical Services - IMS. IMS plan submitted and approved. A fire clearance for 72 children (ages 3 through 5 years old) in classrooms #701, #709 and #710 was granted on July 05, 2022. LPA reviewed with applicant representative the LIC 311A, Records To Be Maintained At The Facility, for child\342\200\231s records, personnel records, administrative records, and documents to be posted.
