What the inspector wrote
On August 22, 2025, at 8:45 AM, Licensing Program Analysts (LPA), Oscar Picazo and Licensing Program Manager (LPM), Rajani Goudreau conducted an unannounced Annual/Random inspection and met with Lead Teacher,
Jackqueline Espinoza Campos. LPA disclosed the purpose of the inspection and toured the facility; indoors and outdoors.
At approximately 9:15 AM, Site Director Ruth Alcantara arrived and continued the tour. LPA accompanied by Site Supervisor toured the indoor and outdoor of the facility. This is a full day program which operates on a
year round
school year schedule. Days and hours of operation are Monday- Friday from 7:00 AM to 6:00 PM. Per the site supervisor, there is currently one (1) classroom in operation. The following ratios were observed:
Infant Room - There were 11 infant children (ages 9 months \342\200\223 24 months) present with one (1) Lead teacher and three (3) instructional assistants.
Furniture and equipment are in good condition. Floors in the facility are clean and safe. There is sufficient napping equipment and meet regulatory requirements. Infant changing tables are within arm\342\200\231s reach of a sink. All toilets and hand washing facilities are in safe and sanitary operating condition. Disinfectants, cleaning solutions, medications and other hazardous items were made inaccessible to children during time of inspection. Per the site supervisor, there are no poisons used or stored within the facility. All kitchen, food preparation and storage areas are clean, free of litter/rubbish and free of rodents. Solid waste storage containers have tight fitting covers and are in good repair. The surface of the outdoor activity space is maintained in a safe condition and free of hazards. Play equipment is in safe condition, free of sharp, loose and pointed parts. Shade in the outdoor play area is provided by a tree and a shade canopy.

Facility has a carbon monoxide detector that meets statutory requirements. The last disaster drill was conducted and documented on 6/30/2025. LPA observed facility uses water activity stations and water sprinkler pads on hot days. Staff were advised to ensure that water stations and sprinkler pads are drained of all water immediately after each use. The site supervisor stated that there are no swimming pools or other bodies of water on the premises and that there are no firearms, ammunition or other weapons allowed or stored on the premises.
The facility provides breakfast, lunch and snack to the children. Menus are posted monthly. Drinking water is available both indoors and outdoors. There are two (2) drinking stations but staff provide filtered drinking water from a portable jug.
The site supervisor was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.
Capacity and limitations as specified on the license are being maintained. Children are under supervision, including visual supervision, of a teacher at all times. Facility maintains a ratio of one teacher supervising no more than three (3) infants in care.
At least one person trained in CPR and Pediatric First Aid is present when children are at the facility. Fully qualified teacher, Jackqueline Espinoza Campos, CRP/First aide expires 12/2025. Mandated Reporter Training was taken on 9/11/2023. The name of the facility representative and the fully qualified teacher designated to act in the facility representative\342\200\231s absence has been reported to the Department. LPA reviewed facility sign in /sign out sheets and based on review, one (1) out of the 12 infants in care was not signed in by the infants authorized representative. LPA informed the site supervisor of the importance of the infants being properly signed in and out by their authorized representative while in care. Additionally, the site supervisor was reminded that the person who signs the child in and out of the facility shall use their full legal signature and record the time of day.
LPA reviewed a sample of children\342\200\231s files and observed files were complete with contact information for authorized representative and or relatives or others who can assume responsibility for the child and medical assessment. Infant Needs and Service Plans and Safe Sleep Logs were reviewed and are being maintained.

This facility provides Incidental Medical Services \342\200\223 IMS, however there are no children receiving services at this time. Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018,
requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test.
For child care center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1-CCP). LPAs verified that the lead testing was completed in accordance to the Written Directives outlined in
PIN 21-21.1-CCP
.
One type B deficiency was issued during this inspection. Per California Code of Regulations, Title 22, Division 12, Chapter 1, deficiencies are being cited on the attached LIC 809D.
A Notice of Site Visit (LIC 9213) was given to site supervisor, Ruth Alcantara and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days. LPAs observed LIC 9213 was posted. Appeal Rights (LIC 9058) was provided. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. LPA observed Notice of Site Visit posted on the wall in the infant classroom.
An exit interview was conducted, and the report was reviewed with the Site Supervisor, Ruth Alcantara.
