What the inspector wrote

On 2/18/2025 at 9:00am, Licensing Program Analyst (LPA), Selina Siao conducted an announced Pre-Licensing inspection for a change of ownership from individual to LLC. Upon arrival, LPA met with Applicant Egle Athari and Director Donna Alcala. Applicant is requesting to be licensed for 10 toddlers (ages 18 to 36 months) in room #3 and 64 preschool children (ages 2 to 5 year old) in classrooms #1, #2, #4, #5, #6 and Art Studio. Facility’s hours of operation is Monday to Friday from 7am to 6pm. All indoor and outdoor activity space utilized for the children was inspected today. LPA informed applicant Egle Athari and Director Donna Alcala that staff are required to maintain direct visual supervision of the children at all times during indoor and outdoor activities. When medications are on site, Egle Athari stated that they will be in a higher cabinet located in the individual’s child’s class. A fully equipped first aid kit is located in the reception area and the hallway that leads to the outdoor playground. There is an operational carbon monoxide detector on site located in the toddler room and the reception area. All required licensing documents will be posted outside the wall next to the new Director’s office. Children will be signed in and out via the Brightwheel app that is available in each classroom. LPA continued to tour the facility and measured all indoor and outdoor activity space. Total toddler indoor activity space measured to be 322.11 square feet which is only sufficient for 9 toddlers insufficient for the requested capacity. The preschool indoor activity space measured to be 2394.79 square feet, which is sufficient to accommodate the requested capacity of 64 preschool children. LPA observed all indoor activity space to be complete with safe, age-appropriate furniture and equipment, including tables, chairs, cubbies, napping cots, bookshelves, and other activity supplies for the children. Drinking water is available in the classrooms via the children’s own water bottles and it gets refill by the water dispenser with five gallons water that the facility orders from Conscious water company. LPA observed all hazardous items to be inaccessible to children. There are no bodies of water or weapons on the property. Fire clearance was granted on 1/22/2025 but LPA needs to request for a new fire clearance due to the changes of the toddler room and include the Art Studio on the fire clearance. LPA observed a total of 1 sink and 1 toilet for the toddler component and there are 7 sinks and 5 toilets for the preschool children’s use. These are sufficient to accommodate the requested capacity. There is a separate staff restroom equipped with a toilet and a sink. The isolation area for children who are ill will be the Director’s office and the staff’s restroom will be use as the isolation restroom. Facility will provide AM and PM snacks to the children. The kitchen area currently includes a refrigerator, dishwasher and a microwave. The kitchen area and food storage areas were observed free of rodents and/or vermin. Hazardous items in kitchen are inaccessible to children via a safety gate. The facility currently has a fully fenced playground area. The wooden fencing and chain link fencing is at least four feet high. The total square footage for all the outdoor activity space measured to be 790.05 square feet for the toddler playground which is sufficient space for the requested capacity. And the preschool playground measured to be 3506.68 square feet, which is sufficient for only 46 children at one time, insufficient for the number of children being requested. A playground waiver to share the playground at different times in the AM and PM due to limited square footage shall be submitted and approved prior to licensure. Shade is provided via multiple sail shades and umbrellas. There are sufficient outdoor age-appropriate toys and play equipment available on the preschool playground. There is a climbing structure on the preschool playground that appears to be age appropriate for preschool children ages 2 to 5 years old. There is adequate cushioning in fall zones of climber provided by rubber cushioning. Drinking water is available via the children’s own water bottles when children are outside. LPA observed all hazardous items on the playground to be inaccessible to children. Applicant Egle Athari was reminded that any changes to the facility must be reported to and approved by Community Care Licensing. For child care center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1- CCP). Applicant was reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5-days or, if the penalty is for a repeat violation, for a maximum of 30-days per person will be assessed if this regulation is violated. This facility plans to provide Incidental Medical Services – IMS. For IMS information, see PIN 22-02-CCP . A Plan of Operation that includes IMS must be submitted to the Department. LPA discussed the safe sleep regulations with applicant and discussed the Child Care Licensing Safe Sleep webpage at: https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep , as an additional resource. LPA also informed [applicant, licensee, or facility representative] of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at: https://www.cpsc.gov/ , and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment. Applicant was informed of the MyChildCarePlan.org site, a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California. Community Care Licensing Division (CCLD) regularly sends information to licensed facilities, providers, and stakeholders by way of Provider Information Notices (PIN), Program Quarterly Update Newsletters and other important information communication platforms. To receive important licensed related information to licensed facilities, visit the CCLD Important Information website at https://www.cdss.ca.gov/inforesources/community-care-licensing/subscribe and select the Child Care option to receive email communication.” The following corrections are needed prior to the issuance of the license: Submit an updated application LIC200a to request for 9 toddlers and 64 preschool children due to the indoor activity space. Clear out the cots in room #6 as it takes up the children’s activity space. Clear out the cleaning items and other products in the staff’s restroom as that is also the isolation restroom for children. Add some age-appropriate toddlers play equipment for the toddler yard. Submit an outdoor playground waiver for the preschool component as the outdoor area is only sufficient for 46 children at one time. Submit an updated LIC500 to LPA to ensure that all staff members are associated to the current pending facility. Updated TB clearance is needed for Donna Alcala. Applicant Egle Athari understands that all proof of corrections must be provided to the Department within 30 days, or the application may be denied. Exit interview conducted and report was reviewed with the applicant Egle Athari.
