What the inspector wrote

On June 2, 2025, Licensing Program Analyst (LPA) Leong conducted an unannounced case management visit to follow up with an incident that occurred on May 6, 2025. LPA met with Director Michie Minamoto and explained the purpose of the visit. Six children, the director, and four staff members were present at today's visit. On May 6, 2025, the facility reported to the San Bruno Regional office regarding an incident involving a child (C1). Staff noticed C1\342\200\231s tooth was chipped and offered C1 ice. According to the director, C1 declined the ice, and the staff reported the incident to the parents on their electronic app, Brughtwheel, and during pick-up. According to the director, C1\342\200\231s parents brought the child to the dentist, and the dentist reported the child was ok. After talking with the director, LPA determined that the staff followed the department\342\200\231s regulation in reporting the incident to the local regional office within one business day and to the parents during pick-up time. The facility also followed the department's regulation by submitting a written report within seven days of the incident. On June 2, 2025, the incident from May 6, 2025, will be closed, and there were no deficiencies cited at this time. A Notice of Site Visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with the facility representative, Michie Minamoto.
