On March 21, 2023, at approximately 10:00am, Licesing Program Analyst (LPA) arrived at facility to conduct a complaint investigation to the above allegation. LPA met with Site Executive Director Erika Tinoco. There were 4 staff including the Directior caring for 19 children during today's visit. During today's visit, LPA interviewed Site Director to confirm information recieved by the department. Based on information received, the preponderance of evidence standards has been met, therefore, the above allegation is found to be SUBSTANTIATED. A Type “A” violation and an Immidiate Civil Penaly of $500 were issued on 03/15/2023. Refer to LIC 809D dated 03/15/2022. This facility will be scheduled for a Non- Compliance conference at the Peninsula Regional Office. A copy of this report and appeal rights were discussed and left with Site Director whose signature on this form confirm receipt of reports. Notice of Site Visit was provided to Director.
