On January 12, 2023 at approximately 9:00am, Licensing Program Analyst (LPA) Catrina Quimbo conducted an unannounced, annual inspection. LPA met with facility's health and safety director, Nedzada Baldwin, and explained the purpose of the visit. Center director was not present during LPA's visit. Health and safety director is designated with facility responsibility. Facility is a combination center that includes an infant, preschool and school age program, each with a separate license. Facility is a Bright Horizons "back up" center that is provided to families that are contracted with family's employers. Hours of operation are Monday to Friday 8:00am to 6:00pm. Under preschool license, there is 1 staff member present with 1 preschool age child. On site there are 3 additional staff members (including health and safety director) and 4 infants (in infant classroom) present. Facility is operating within capacity limits and ratio during LPA's visit. With health and safety director, LPA inspected entire facility for health and safety hazards. Preschool program operates in one classroom. LPA observed preschool classroom to have age appropriate toys and materials that were in good working condition. Classroom is equipped with age appropriate furniture and bedding. Mats are utilized for napping children. Per health and safety director, sheets are laundered after each use. Classroom also includes two additional sinks and a separate bathroom. Sinks accessible to children to do not provide water that is warmer than 120 degrees Fahrenheit. LPA observed bathroom located in classroom to also have a curtain for additional privacy for older preschool age children. Bathroom was observed to be in proper working condition, equipped with appropriate sanitation products. Storage for children's belongings are located in the classroom, labeled with each child's individual names. Facility provides food service to enrolled children that include morning and afternoon snack only. Children's families are required to provide children's lunches daily. LPA inspected food prep area that was observed to be clean and organized. All food is stored and prepared properly to avoid contamination. (Continue Report on Page 2...) (Continued, Page 2...) LPA observed facility has multiple working smoke and carbon monoxide detector and fully charged fire extinguishers. Facility has a working telephone on site. LPA observed all poisons, cleaning solutions and hazardous materials are stored inaccessible to children in classrooms' high shelves. Preschool program shares "outdoor area" / multi-purpose classroom with infant and school age programs on a rotation schedule. LPA observed multi-purpose area to be equipped with sufficient toys and materials that were in good working condition. Water is made available to children with their own individually labeled water bottles that can be refilled at facility when needed. Facility completed required lead water testing that was submitted to department 04/2022. Children are signed in and out of facility by authorized representatives' written signatures. Facility has required license documentation and information properly posted and available for review. LPA reviewed facility records that included 4 children's records and 4 staff records (all of whom were present during LPA's visit). LPA observed children's and staff's records to be complete. Health and safety director's CPR and First Aid certificate is current and will expire 04/2023. Facility's last emergency disaster drill was conducted 01/09/2023. Emergency drills are conducted at least once every six months and are properly logged. Facility was informed that as of September 1, 2016, a person may not be employed or volunteer at a child care facility unless he or she has been immunized against influenza, pertussis, and measles or qualifies for an exemption pursuant to Health and Safety code 1596.7995 and 1597.662. Health and safety director is aware that all staff is required to complete Mandated Reporter Training every two years. The training can be obtained online at www.mandatedreporterca.com . LPA observed the completion certificates of all site on site, on file. To improve the quality and value of the new inspection process, a survey will be sent to the email address provided. Health and safety director was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated. LPA discussed the safe sleep regulations with director and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed director of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment. Incidental Medical Services (IMS) policy was discussed. Program is not providing IMS at this time. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
