Facility-requested visit·May 30, 2025

On May 30, 2025, at approximately 1:00pm, L icensing Program Analyst (LPA) Melissa Zaragoza conducted an unannounced, case management visit. LPA met with the licensee Gilma Bardales Rodriguez and explained the purpose of the visit. Present during LPA's visit included the licensee, licensee's spouse, and 4 children (2 infants and 2 preschool age). Licensees requested for a room addition to their childcare license. The licensee submitted an application for an update to their license, to department on May 21, 2025. Licensee, lives in the home with their spouse. All adults living and working in the home have fingerprint clearance on file. Hours of operation are Monday through Friday 8:00am to 4:00pm and 4:00pm to 10:00pm. The DAY CARE AREAS now approved are first level: bedroom #1, bedroom #2, kitchenette/family room, bathroom #1, bathroom #2, and the backyard. The OFF LIMIT AREAS are the garage, laundry area, and the entire second level of the home. Off limit areas are made inaccessible with child safety door handles and/or a child safety gate. Home is equipped with a fully charged fire extinguisher, first aid kit and a smoke and carbon monoxide detector. LPA observed smoke and carbon monoxide detector in the first level of the home. LPA observed the back yard of the home and first level, for any health and safety hazards. Back yard has a wooden fence enclosing the yard. First level of the home has been equipped with playpens for napping children, a changing table, and appropriate dining tables and chairs for children to use for eating. LPA also observed outlets to have outlet covers. LPA observed the backyard and the first level of the home to have many toys and material that were age appropriate for the children in care. LPA observed toys to be well kept and in proper working condition. Licensee installed corners covers for protection on furniture, and in outdoor materials. LPA did not observe any pools, spas, or bodies of water in the premises. (Continue Report on Page 2...) (Continued, Page 2...) LPA will approve the room addition of the back yard, First Floor: bathroom #1, bathroom #2, kitchenette/family room, bedroom #1, bedroom #2 to the childcare license, as of today\342\200\231s date, 05/30/2025. No deficiencies were issued during today's visit. A notice of site visit was given and must remain posted for 30 days.
