On September 15, 2025, Licensing Program Analyst (LPA) Stacey Williams met with Facility Representative, Isabella Mendoza for the purpose of a case management inspection. LPA observed thirty nine children supervised by 7 staff. LPA discussed an unusual incident report submitted to Community Care Licensing involving a child with a medical alert. LPA toured the facility with the Facility Representative and reviewed C1's file which included a safety plan in place that was followed during the incident. Facility Representative informed LPA that staff are familiar with C1's medical condition and trained. C1's medical binder is in each preschool classroom. Exit interview conducted and report was reviewed with the Facility Representative, Isabella Mendoza. A notice of site visit was provided and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. Appeal Rights were provided.
