What the inspector wrote

Licensing Program Analyst (LPA) David Nguyen met facility representative, Shannon Rodriguez, to conduct a proof-of-correction inspection to verify corrections of one (1) Type-B deficiency and two (2) Type-A deficiencies cited on September 5 th , 2025. The purpose of the proof-of-correction inspection was explained. There were thirty (30) childcare children present being supervised by five (5) childcare teachers during today\342\200\231s proof-of-correction inspection. On September 5 th , 2025, the childcare facility was cited for one (1) Type-B deficiency for not completing the child care provider mandated reporter trainings. During the inspection on September 5 th , 2025, LPA reviewed staff records, and based on record reviews, staff members did not have valid child care provider mandated reporters in the files. In addition, the childcare facility was cited for two (2) Type-A deficiencies for not keeping the childcare center safe and in good repair and for leaving a childcare child without supervision of a childcare teacher. During the inspection on September 5 th , 2025, LPA observed one (1) window air conditioner when in operation dripping water excessively on the playground, which was accessible to childcare children, and LPA observed a childcare child was left unattended in the bathroom as the child was washing hands. During today\342\200\231s inspection, LPA reviewed teachers\342\200\231 files and children\342\200\231s files. LPA observed teachers\342\200\231 valid child care provider mandated reporter certificates in teachers\342\200\231 files and signed LIC9224s\342\200\224Acknowledgement of Receipt of Licensing Report in children children\342\200\231s files. LPA, in addition, observed the window air conditioner, which was dripping condensation water excessively in the playground, has been replaced. Report continues on LIC809-C... (Page 2) (Page 2) Furthermore, LPA obtained the teachers\342\200\231 meeting agenda, which stressed bathroom supervision, held on September 8 th , 2025, as well as the teachers\342\200\231 meeting sign-in sheet from the facility representative during today\342\200\231s inspection. While LPA toured on-limits areas, LPA observed supervision and ratios were appropriate. The Type-B deficiency cited on September 5th, 2025, for not completing the child care provider mandated reporter trainings was cleared on September 11th, 2025. In addition, the two (2) type-A deficiencies cited on September 5 th , 2025, for not keeping the childcare center safe and in good repair and for leaving a childcare child without supervision of a childcare teacher were also cleared on September 11 th , 2025. The Proof-of-Correction Letters were provided. In the areas that were evaluated today, no deficiencies were observed or cited. An exit interview was conducted with facility representative, Shannon Rodriguez. Appeal rights and a Notice of Site Visit were provided. The facility representative\342\200\231s signature on this form acknowledged the receipt of the form. A notice of site visit was posted by LPA and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
