Interviews conducted, revealed inconsistent information regarding the supervision at the facility. Although C1 potentially sustained injuries, it was not conclusive it was due to a lack of supervision. Based upon interviews conducted and observations while at the facility, there is not a preponderance of evidence to prove or disprove the allegation did or did not occur, therefore the above allegation is found to be UNSUBSTANTIATED. The report was reviewed with the Director, Melissa Prouty and an exit interview was conducted. Appeals of Rights and Notice of site visit were provided. Notice of Site visit to be posted for 30 days.
