Inspector’s narrative

What the inspector wrote

Licensing Program Analysts (LPAs) Janie Davis and Lauren Scott met with Director, Dawn Briggs for an unannounced Case Management Inspection. LPA observed 35 children being cared for by three staff members. The purpose of today\342\200\231s inspection was to follow up regarding a water sampling that indicated Action Level Exceedance (ALE) in an outlet tested for lead levels on 3/2025. The facility was notified that water levels were in exceedance of 5 parts per billion in a single playground faucet. Since receiving the notification Director placed the fountain off limits and use water bottles while outdoors and utilizing the area, and are waiting the required amount of time to have it retested. Director indicated that they do not utilize the yard with the fountain in it; however, they will not use the fountain until it is retested and determined to be safe. Director advised LPA that they have water bottles and other fountains available for drinking water. During today\342\200\231s visit LPA confirmed that the fountain had been placed off limits. LPA informed Director that Grant funding for testing and remediation is available and that information could be found in Provider Information Notice (PIN) 21-04-CCP. A Type B deficiency was cited on the subsequent page (LIC809-D) of this report. An Exit interview conducted, and the report was reviewed and provided to Director. LPA posted the Notice of Site Visit and Director acknowledges it must remain posted for 30 days. A Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
