Follow-up on corrections·August 19, 2025

This is an amended version of the report created on 08/19/2025. On 08/19/2025, Licensing Program (LPA) Janie Davis conducted a field visit to the facility for the purpose of opening a complaint. During that visit, a case management inspection was also conducted. LPA arrived at the facility and was met by Facility Representative Sarah Orian. LPA disclosed the purpose of the inspection and was granted entrance. LPA noted the census of 15 children present. LPA determined through accessing Guardian that all required adults were background cleared and associated to the license. An exit interview was conducted, and the report was reviewed with Facility Representative Elizabeth Langum. LPA provided Appeal Rights. A Notice of Site visit was posted by LPA and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
