On November 5, 2025, Licensing Program Analyst (LPA) Lauren Scott met with facility representative, Stephanie Hodges for an unannounced Case Management Inspection. The purpose of today\342\200\231s inspection was to follow up regarding a water sampling that indicated Action Level Exceedance (ALE) in an outlet tested for lead levels on 3/2025 that was cited on 5/2025. LPA Scott confirmed the water fountain has been removed entirely. Since there is no access to the fountain any longer, retesting of the outlet, is not necessary. LPA informed facility that Grant funding for testing and remediation is available and that information could be found in Provider Information Notice (PIN) 21-04-CCP. LPA also reminded facility to retest outlets every five years. An Exit interview conducted, and the report was reviewed and provided to Facility Representative, Stephanie Hodges. LPA posted the Notice of Site Visit and facility acknowledges it must remain posted for 30 days. A failure to comply with posting requirements shall result in an immediate civil penalty of $100.
