What the inspector wrote

On April 16, 2026, Licensing Program Analyst (LPA) Stacey Williams met with Facility Representative, Marti Rhoades for the purpose of a case management inspection. LPA observed seven children supervised by two staff. On April 8, 2026, Community Care Licensing was notified of an incident involving child 1 leaving the classroom. LPA discussed the incident with Facility Representative. It was reported that staff was in the classroom supervising seven children when C1 was observed running out the classroom without permission. Staff present ran after C1, which left the children unattended in the classroom for under one minute. Based on the information received, 1 type A Title 22 deficiency will be cited on the attached LIC 8099D for a lack of supervision. Upon receipt of Type A citations, facility shall post and provide copies of the LIC 809D for parents/guardians of children currently in care and for parents/guardians of newly enrolled children for the next 12 months. Facility must also keep the signed LIC 9224, Acknowledging Receipt of Licensing Report LIC 809D in each child's files. Exit interview conducted at which time the report was reviewed with Facility Representative, Marti Rhoades . A Notice of Site Visit was posted by LPA Williams and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.00.

  • RESPONSIBILITY FOR PROVIDING CARE AND SUPERVISION

    No child(ren) shall be left without the supervision of a teacher at any time, except as specified in Sections 101216.2(e)(1) and 101230(c)(1). Supervision shall include visual observation. This requirement was not met as evidenced by: staff was in the classroom supervising seven children when C1 was observed running out the classroom without permission. Staff present ran after C1, which left the children unattended in the classroom for under one minute. This is an immediate risk to the health and safety of children in care.
