What the inspector wrote

The allegation is determined to be substantiated. Title 22 deficiencies will be cited on subsequent page, LIC 9099D. Exit interview conducted at which time the report was reviewed with Facility Representative, Marti Rhoades. A Notice of Site Visit was posted by LPA Williams and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.00.

    PERSONAL RIGHTS

    (a) The licensee shall ensure that each child is accorded the following personal rights:To be free from corporal or unusual punishment, infliction of pain, humiliation, intimidation, ridicule, coercion, threat, mental abuse or other actions of a punitive nature including but not limited to: interference with functions of daily living including eating, sleeping or toileting; or withholding of shelter, clothing, medication or aids to physical functioning. This requirement was not met as evidenced by: C1 was yelled at by S1 and had a pillow hit them as S1 removed the pillows. This is a risk to the health and safety of children in care.
