What the inspector wrote

On December 29, 2025, at 10:30 AM Licensing Program Analyst (LPAs) Deborah Khashe and Sarah Tibbett met with licensee, Ileana, Castillo Diaz for the purpose of an unannounced visit. The purpose of the visit was explained and a copy of Entrance Checklist was provided. During todays inspection there were two (2) children present. Facility hours of operation are Monday \342\200\223 Friday from 7:00 am \342\200\223 5:00 pm. LPA verified that annual fees are current. Facility Representative, Brianna Diaz was reminded that all adults 18 and over living or working in the home, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated. A health and safety inspection was conducted in all areas accessible to children. Off-limits NONE. LPA observed the required postings and a working phone. 3A40BC fire extinguisher meets regulations. LPA observed functioning smoke and carbon monoxide detectors. LPAs toured the kitchen area and verified knives were inaccessible to children in care. LPAs toured kitchen and verified that chemicals were accessible to children in care. LPA observed playroom area with age-appropriate toys for children. LPAs observed a restroom and verified that hazardous and toxic items were accessible to children in care. Licensee stated there are no weapons in the home. Outdoor play space is fenced. There are no bodies of water on the premises. Report Continues on LIC 809C... (page 2) Page 2 5 Children's files were reviewed. Emergency information and required immunization records were on file. Current EMSA pediatric CPR and First Aid certification was verified and expired and Child Care Provider Mandated Reporter certification was verified and expired. LPA discussed the safe sleep regulations with licensee and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed licensee of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment. LPA discussed the requirement to check and log infant napping every 15 minutes for infants 24 months and under. LPA discussed LIC 9227 Individual Sleeping Plan for infants under 12 months. Licensee states that she is not caring for infants at this moment. This provider is not currently providing Incidental Medical Services (IMS) services to children in care. IMS policy was discussed. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. Report Continues on LIC 809C...(Page 3) Page 3 Licensee understands that licenses are not transferable, and once licensed, licensee must live in the home and be present for 80% of the operating hours. Applicant understands that if an unusual incident occurs, licensing is to be notified via phone call, e-mail or fax within 24 hours, and the Unusual Incident Report LIC 624 shall be submitted within 7 days to remain in compliance. Licensee understands that if any structural changes are made to the home, licensing must be notified prior to construction On this date, 12/29/2025, the California Attorney General - Megan\342\200\231s Law website was searched for information on sex offenders required to register with local law enforcement under California's Megan's Law. No registered sex offenders were found at the facility address. Under state law, some registered sex offenders are not subject to public disclosure; therefore, they may not have been included in this search. However, the Department conducts a monthly cross reference of each address on record for all registered sex offenders against all CCLD facility addresses pursuant to information shared by California DOJ. Facility Representative was informed of the MyChildCarePlan.org site, a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California. Community Care Licensing Division (CCLD) regularly sends information to licensed facilities, providers, and stakeholders by way of Provider Information Notices (PIN), Program Quarterly Update Newsletters, and other important information communication platforms. To receive important licensed related information to licensed facilities, visit the CCLD Important Information website at https://www.cdss.ca.gov/inforesources/community- care-licensing/subscribe and select the Child Care option to receive email communication. A notice of site visit was given to Licensee, Ileana Castillo Diaz and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days. Exit interview conducted and report was reviewed with the Licensee, Ileana Castillo Diaz . Appeal Rights were provided. Page 4 LPA Khashe informed licensee, Ileana, Castillo-Diaz that this report dated (12/29/2025) document(s) 2 Type A citations which shall be posted for 30 consecutive days as there is/are immediate risk(s) to the health, safety, or personal rights of children in care. Also, LPA Khashe informed the licensee, Ileana, Castillo-Diaz to provide a copy of this licensing report dated 12/29/2025 that documents any Type - A citation(s) to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification. Failure to comply with posting requirements will result in an immediate civil penalty of $100. A notice of site visit was given to Licensee, Ileana Castillo Diaz and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days. Exit interview conducted and report was reviewed with the Licensee, Ileana, Castillo-Diaz . Appeal Rights were provided. In the areas that were evaluated, there were 2 Type A deficiencies and 2 Type B deficiencies cited during today\342\200\231s inspection.
