101229 Responsibility for Providing Care and Supervision (a) The licensee shall provide care and supervision...(1)...Supervision shall include visual observation.This requirement is not met as evidenced by: Based on observations, interviews conducted, and records reviewed, it was revealed that C1 sustained an injury resulting in a bruise on their cheek while in care at the facility that could not be explained by staff which poses an immediate health, safety, or personal rights risk to children in care.

This requirement was not met as evidenced by: On 9/19/22 video evidence revealed that C1 did not have a diaper change for approximately 5 hours. The video also showed the teacher's failure to wash hands in between or after diaper changes. This is a potential health and safety risk to children in care.
