What the inspector wrote

On September 15, 2025, Licensing Program Analyst (LPA) Stacey Williams met with Director Alicia Neves for the purpose of a case management inspection. LPA observed twenty seven children supervised by 6 staff. LPA discussed an unusual incident report submitted to Community Care Licensing involving an injury to a child. LPA toured the facility with the Director. Director informed LPA of precautionary measures that have been taken as result of the incident. Exit interview conducted and report was reviewed with the Facility Representative, Alicia Neves. A notice of site visit was provided and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. Appeal Rights were provided.
