What the inspector wrote

On February 14, 2023 Licensing Program Analysts (LPAs) Lauren Scott and Tobias Lake conducted an unannounced case management inspection at Zion Child Care Center . The purpose of the inspection is to verify that the required removal of the individual who was not cleared to work at the facility took place. LPA has confirmed Sari Lindsey has been removed and is not working at the facility. LPAs verified that all staff present today has a criminal record clearance. LPAs toured/inspected the facility inside and out. Based on evidence obtained during today's inspection, the LPA has verified that the individual is not present, employed, or residing at the facility. LPA verified the individual has been separated from the facility's Guardian roster by the facility. LPA received an updated copy of the LIC500. As a result of today’s inspection, no Title 22 deficiencies were cited. Report was reviewed with the director, Jennifer Sawyer. A notice of site visit was provided and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. Verification of removal is complete.
