Inspector’s narrative

What the inspector wrote

On March 23 rd , 2023, at 10:55 am, Licensing Program Analysts (LPAs) Lauren Scott and David Nguyen met with Site Supervisor, Nadia Khan, for the purpose of an unannounced Annual inspection. LPAs toured the facility inside and out. LPAs observed that hazardous items (disinfectants, cleaning solutions etc.) were inaccessible to children in care. Facility days and hours of operation are Monday-Friday from 6:30 AM to 6:00 PM. Facility prepares meals on site and provides breakfast, lunch and snack. There were twenty nine (29) children and three (3) teachers present at the start of today's inspection. LPAs reviewed staffing ratios, first aid supplies, furniture, equipment, fire drills and drinking water. LPAs observed all required forms to be posted. LPAs observed functioning carbon monoxide and smoke alarms. There are adequate toys and equipment available for children. Outdoor play area was toured, the play structure appeared to be in good repair, and there is sufficient cushioning (wood chips) under the play structure. LPAs reviewed the sign in/out electronic printouts and observed that the children were signed in properly. LPAs reviewed children’s and staff files. LPAs observed health screening reports with TB test and required immunization's. At least one staff member present today has current Pediatric CPR and First Aid (exp. 11/2024). LPA discussed the requirement of the AB1207 Mandated Reporter training certificates for all staff. The Site Supervisor was reminded to renew the course every 2 years through www.mandatedreporterca.com website. Site Supervisor was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated. Report continues on 809-C. This facility provides no Incidental Medical Services (IMS) during today’s inspection. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. Exit interview conducted and report was reviewed with the site supervisor, Nadia Khan. A notice of site visit was posted by LPA Nguyen and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. LPA Scott informed site supervisor, Nadia Khan that this report dated March 23, 2023 documents one Type A citation. Type A citations which shall be posted for 30 consecutive days as there is an immediate risks to the health, safety, or personal rights of children in care. Also, LPA Scott informed site supervisor, Nadia Khan to provide a copy of this licensing report dated March 23, 2023 that documents any Type A citations to parents/ guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.
