What the inspector wrote

(Page 2) In addition, based on interviews with childcare staff and the reporting party and records obtained during the course of investigation, LPA obtained corroborating information from the childcare staff as well as the reporting party, and records revealed that the childcare staff did not fully provide the child’s authorized representative with the child’s record upon request. Based on corroborating information from childcare staff and reporting party and record reviews, there is a preponderance of evidence to prove the alleged violations did occur at the facility. Therefore, the above allegations are found to be SUBSTANTIATED . The following Title 22 Deficiency is being cited on the subsequent 9099-D page. An exit interview was conducted, and a copy of this report was provided and reviewed with the facility representative, Guadalupe Espinoza. Appeal Rights and a Notice of Site Visit were provided. A notice of site visit was provided and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
